Welcome to the COPDGene Study Survey

Thank you for participating in the COPDGene study, one of the largest studies ever funded by the National Heart, Lung, and Blood Institute of the National Institutes of Health. Your participation in COPDGene is helping us get a better understanding of how to manage, treat, and possibly cure COPD in the future. In order to accomplish this goal, we like to stay informed on your health status by requesting you complete the COPDGene study survey up to four times a year. The survey is fast and easily accessible on the web or by phone.

If you would rather take the survey by phone, please call us toll free at 866-706-1270. You must call the COPD line from one of the phone numbers that you gave us on your initial study visit. Our automated system will link the phone number you call from with your information so that we know who is taking the survey.

Completing the survey online is fast and simple. Just click the Take Survey button below and you will automatically be directed to the survey. You will be asked to provide your user name (e-mail address) and password to log into the survey.
